Transitional Care

LifeCare Health Partners specializes in caring for patients who may benefit from transitional skilled nursing care as they progress to a lower care setting or back into their home.

Patients may be admitted following a hospital stay or through a referral from another healthcare provider, such as a primary care physician or home health care agency.

LifeCare’s highly trained team manages patients requiring continued nursing, physical, occupational, speech and respiratory therapies in direct collaboration with on-site medical staff. Physician and nursing teams work closely with patients and families to develop individualized care plans with the ultimate goal of maximizing strength, mobility and balance to restore the highest level of functional independence.  

Transitional care services are currently offered at LifeCare Hospitals of Pittsburgh in the Allegheny General Hospital Suburban Campus.  The transitional care facility is located on the same site as the hospital, allowing the LifeCare team to provide a continuum of care to patients whose recovery will benefit from expert inpatient or specialized post-acute care.
