Our home health team consists of registered nurses, licensed vocational/practical nurses, physical therapists, occupational therapists, speech therapists, medical social workers and certified home health aides. We take time and consideration to match you with the nurse and therapist who have the skills and expertise to meet your needs.
Skilled Nursing
HOME HEALTH TEAM
LifeCare nurses are licensed in their state of practice. All our nurses undergo extensive ongoing specialty training designed to address home health care concerns.
The LifeCare nurses will work with you, your caregivers and your doctors to develop plan of care designed to meet your needs. This plan will include the right combination of services to address your condition. Being a LifeCare patient means that you’ll have access to on-call support when you need it, 24 hours a day, seven days a week.
The goal of home health care is to maximize your physical, cognitive and behavioral well-being so you can live independently at home and in your community. Your RN case manager coordinates, oversees and manages your care plan and all caregivers involved in your care to ensure your care needs are met:

Therapy Services
HOME HEALTH TEAM
Our highly trained physical therapists, occupational therapists and speech-language pathologists combine the latest medical knowledge and most up to date technologies with best-practice techniques and hands-on care to make sure you receive the exact treatment you need. Your therapist will work with you to develop goals for your recovery and will work with your doctor and your LifeCare nurse to create a plan of care that helps you develop the skills and confidence your need to regain independence.
Physical Therapist
HOME HEALTH TEAM
Your LifeCare physical therapist will work with you as you recover from surgery, injury and hospitalization so that you can become self-sufficient and return to everyday life. Your physical therapist will:

Determine whether you would benefit from a cane or walker and make sure you know how to use it safely and effectively

Suggest ways you can conserve energy and limit strain on joints and muscles

Make sure you can get in and out of bed or chair safely

Occupational Therapist
HOME HEALTH TEAM
Occupational therapy can help you regain the ability to perform everyday activities, such as eating, dressing, bathing, writing or enjoying a favorite hobby. Occupational therapy is beneficial after a physical injury or stroke or for people with brain cancer, neurological conditions, diabetes, arthritis or other serious long-term illness. Life are occupational therapists visit you at your home to help you to:

Speech Therapy
